Please note
CEDR Solve will nominate an adjudicator within 5 days (excluding bank holidays) on receipt of this form, copy of Notice of Adjudication and fee. The appointment does not form a contract with the Adjudicator with whom conditions of appointment and fees must be agreed directly.
